Objective Analysis of 7 Biomarkers

When diagnosed with diabetes, usually one of the following occurs: the pancreas does not product any insulin or very little insulin and the body does not respond to insulin. According to World Health Organization (WHO), people are diagnosed with diabetes when their fasting glucose level is greater than or equal to 126 ml/dL or their 2hr-postprandial glucose level is greater than 200 mg/dL, and people are diagnosed with pre-diabetes when their fasting glucose is between 100 ml/dL and 125 ml/dL or 2hr-postprandial glucose level is between 140 ml/dL and 200 ml/dL. Therefore, these two biomarkers, fasting glucose and 2hr-postprandial glucose, are most commonly used.

These two biomarkers do not present a complete status of diabetes patients. They provide an accurate measurement of glucose level, but not that of insulin levels either produced by the body or injected. They also do not indicate any previous history of glucose levels. Although diabetes patients need to ultimately control their glucose levels, other important biological functions may need to be controll
ed first. The two biomarkers may need time to improve for a given medication before the drug is abandoned. Many wonderful drugs may need time to take effect. For instance, they may first increase the production of insulin after food intake. Other more sensitive biomarkers are needed for early indication of the body's response for a given drug.

In order to get a complete status of a diabetes patient and to provide a sensitive measure of a patient response to a given drug, we propose a 7-biomarkers system. They are:

  1. Fasting glucose
  2. 2hr-postprandial glucose
  3. Fasting insulin
  4. 2hr-postprandial insulin
  5. Fasting c-peptide
  6. 2hr- postprandial c-peptide
  7. Hemoglobin A1c (HbA1c)
The first and second measure the glucose level, the third and fourth measure the insulin levels, the fifth and sith measure the insulin produced by the body and the last measures the history of glucose levels in previous 3 months. The ratio of 2hr-postprandial insulin over fasting insulin, and 2hr-postprandial c-peptide over fasting c-peptide, represent the body's response to the intake of glucose. The ratio of 2hr-postprandial glucose over fasting glucose measures the body's ability to glucose surge after meal. These ratios are early indicators of a drug's effect. We may get more timely information from 7-biomarkers system than from 2-biomarkers system normally used by healthcare practitioners. It is suggested that the diabetes patients to follow the instructions of their healthcare providers to measure their 7-biomarkers.
